# Terms of Service **Last updated:** September 24, 2026 These Terms of Service govern your use of our website and any products or services purchased from Peak Wellness Co ("Peak Wellness Co", "we", "us"). ## 1. Who we are Peak Wellness Co operates a wellness storefront. Consultations, prescribing decisions, and pharmacy dispensing are performed by independent, licensed healthcare providers and pharmacies. We do not practice medicine. ## 2. Eligibility You must be at least 18 years old and located in a state where the requested product or service is available. You agree to provide accurate health and shipping information. ## 3. Orders and pricing Prices are shown at checkout in U.S. dollars and may change. An order is only accepted once payment clears and, where required, a licensed provider approves treatment. If a provider declines treatment, we will not charge you for the product. ## 4. Consultations and prescriptions Where a product requires a prescription, you must complete a health intake and consultation. The decision to prescribe rests solely with the licensed provider. Lab work may be required before treatment begins. ## 5. Shipping Products ship from licensed pharmacies. Delivery timelines are estimates. Risk of loss passes on delivery to the carrier. ## 6. Refunds Refunds are governed by our Refund & Shipping Policy. ## 7. Acceptable use You may not resell products, misrepresent your identity or health history, or use the site unlawfully. ## 8. Limitation of liability To the maximum extent permitted by law, Peak Wellness Co is not liable for indirect, incidental, or consequential damages.
